Move mouse hiding code to check_events, this allows mouse hiding
to continue to work with -idle -fixed-vo, after the video has
finished but the last frame is still displayed.

Send KEY_CLOSE_WIN instead of KEY_ESC when the user requested

Signal() has an unavoidable race-condition on "broken by
backwards-compatibility" systems like Solaris. (Upon receiving a
signal, the handler is reset to SIG_DFL, thus a second signal will
kill the process. The problem could also be reduced by re-installing
the handler inside the handler, but there's still a race-condition and
the risk of the handler being called inside the handler).

Add support for parameter changes (e.g. channel count) during playback.
This makes decoding AC3 files that switch between 2 and 6 channels
work reasonably well even with -channels 6 and ffac3 decoder.

Allocates a new sh_sub and specifies language at the same time. I
don't think this is a particularly good API but add it for easier
compatibility with some svn changes.
