Remove the private bswap and intreadwrite.h implementations and use
libavutil headers instead.
Originally these headers weren't publicly installed by libavutil at
all. That already changed in 2010, but the pure C bswap version in
installed headers was very inefficient. That was recently (2011-12)
improved and now using the public bswap version probably shouldn't
cause noticeable performance problems, at least if using a new enough
compiler.

Replace sizoef(type) by sizeof(*ptrvar).
Besides being consistent with FFmpeg style,
this reduces the size of a patch to rename these
types to not conflict with the windows.h definitions.

Current FFmpeg installs intreadwrite.h, but keep using the internal
version for now to keep the possibility of compiling against older
FFmpeg lib versions.
Nowadays MPlayer does not use the codecs from other installed programs.
A runtime setting will soon take over the rare case that binary codecs
should be searched for in non-standard directories.

Note that r30455 is wrong, that commit does not in fact change the
default behavior as claimed in the commit message. It only breaks
"-af-adv force=0", which was already pretty much useless though.
The functions are required to load binary Real codecs, so they need to be kept.
Group all functions together in ad_realaud.c because all the functions are used
to load audio binaries and only some to load video binaries.
