default features: make musl usable without feature test macros
...
the old behavior of exposing nothing except plain ISO C can be
obtained by defining __STRICT_ANSI__ or using a compiler option (such
as -std=c99) that predefines it. the new default featureset is POSIX
with XSI plus _BSD_SOURCE. any explicit feature test macros will
inhibit the default.
installation docs have also been updated to reflect this change.

patch by Isaac Dunham. matched closely (maybe not exact) to glibc's
idea of what _BSD_SOURCE should make visible.

add one more bogus legacy header
...
this one is for program(s|ers) who haven't heard of uint16_t and
uint32_t (which are obviously the correct types for use in such
situations, as they're the argument/return types for ntohs/htons and
ntohl/htonl).

resolve some header namespace non-issues
...
after re-reading 2.2.2 of POSIX 2008, all of these are in the correct
reserved namespaces and do not need protection.
