- fix for growing avi files (movi_end pos > stream->end_pos)
- implement forward seeking in avi streams with no index

mencoder:
- finish mencoder -ovc vfw (bitrate setting, codec selection etc)
- add ogg/vorbis audio encoder
- stop/resume
