960b978a03 Fix video delay when encoding with B-frames. 
Encoding delay is adjusted for on-the-fly during encoding.

Decoding delay is compensated for by setting an appropriate dwStart on
the audio stream (only in muxer_avi at this point).

b4e5108496 dwStart support for mplayer. 
* demux_avi sets stream_delay according to dwStart
* mplayer adjusts audio_delay according to stream_delay

3 related patches are pending, and I will apply them over the next few
days if all goes well.

As I mentioned on -dev-eng, the funny indentation is in preparation for
one of those pending patches.

1ab575101a Second-try commit of this patch. 
1. Include audio_delay as an argument to demux_seek.
2. Modify demux_seek_avi to adjust the audio/video stream positions so
   that mplayer/mencoder will instantly be in sync even when -delay is
   specified.

I've quadruple checked this time; hopefully I haven't missed anything.

955fbdff3f we cannot continue without a crash when read_asf_header fails, since some 
memory is allocated in that function (ugly!!).

e7db4ccf1a Patch by Stefan Huehner / stefan % huehner ! org \ 
patch replaces '()' for the correct '(void)' in function
declarations/prototypes which have no parameters. The '()' syntax tell
thats there is a variable list of arguments, so that the compiler cannot
check this. The extra CFLAG '-Wstrict-declarations' shows those cases.

Comments about a similar patch applied to ffmpeg:

That in C++ these mean the same, but in ANSI C the semantics are
different; function() is an (obsolete) K&R C style forward declaration,
it basically means that the function can have any number and any types
of parameters, effectively completely preventing the compiler from doing
any sort of type checking. -- Erik Slagter

Defining functions with unspecified arguments is allowed but bad.
With arguments unspecified the compiler can't report an error/warning
if the function is called with incorrect arguments. -- Måns Rullgård

b59db28355 This patch makes real rtsp tell the server to deliver data at specified 
bandwidth, if bandwidth cmdline option is given. Also, if that's given,
it uses it for sdp stream selection. If not given, the behavior is the
same as before.

It's used to implement something like turboplay in realplayer.

ffb7208370 Partial support for QuickTime sound atom version 2. 
This doesn't add support for parsing the sound atom itself, but does
recognize the different offset at which the ESDS atom starts. Also,
this patch supports "3" in the channels field, which indicates

c4f58eee9e muxer_lavf MUST be disabled by default until someone adds AVParser 
support to it. Otherwise users will generate files with totally
nonsensical pts if they use B frames! (And they are already doing so
-- see mplayer-users list!)

If anyone wants to volunteer to add AVParser support, go right ahead!
But until then, do not remove this check. :)

eeaa5eb45f set cdda paranoia default to 0 since e.g. cdda://2 breaks otherwise. 
this was the default before, too, since a function call was missing and
thus the paranoia setting was ignored

754c2b98ef Fix two (loosely) related bugs: massive A-V desync with -audiofile (bugzilla 
bug #375, sh_audio->delay must only be set when seeking, not at every packet)
and not switching to the next file when seeking hits eof.

3892465935 processing audio is sometimes essential for a/v sync, so 1000l to 
whoever made rawvideo muxer disable audio!!

with this patch, audio is processed but simply thrown away by the
muxer. various 'error' conditions in rawvideo muxer are removed to
make it work. feel free to re-add them if they can be done without
breaking anything, but do not use printf !!!!

btw old behavior can be obtained by manually specifying -nosound.

b136fa417f makes demux_lavf (-demuxer 35) use the framerate specified in the container 
if it's set and only fall back to the codec framerate if the former is not
set.
This solves the issue of some avi's playing at 30000/1 fps instead of the
correct framerate.

f1e016fb24 the on suboption of -rawaudio and -rawvideo do not work anymore since the
