Create a fmt-conversion.c file so fmt-conversion.h can be included by multiple files.

Also restructure the code so it provides a pixfmt2imgfmt function, too.

DOCS/tech/colorspaces.txt says I420 and IYUV are the same, so add IYUV at the

same place. It can't break anything worse than it is now (i.e. not working/supported
at all) and hopefully triggers a bug report so it gets fixed if it's wrong ;-)
