This should not happen at all.

During several millions of operations hour, it occurs however when
hardware is defective. Try self-healing as far as possible.

from the original author as well as parts from 1&1.

to put their files also under GPL or FDL, or at least a compatible
license (see newest descriptions at fsf.org).
