Some callers assume that item_name is always set, so this may be
considered an API break.

Unnecessary since acf63d5350adeae551d412db699f8ca03f7e76b9;
also avoids relocations.

Look for the generic "USR" labels instead of "?" to skip channels with no
known names, and actually print the decomposition of standard channel layouts.
