Martin Storsjö 4fa57d524f libavformat: Set the default for the max_delay option to -1 
Make the muxers/demuxers that use the field handle the default
-1 in the same way as 0.

This allows distinguishing an intentionally set 0 from the default
value where the user hasn't set it.

Janne Grunau af9240cd3c mpegenc: simplify muxrate calculation 
The fate-h264-bsf-mp4toannexb failures were caused by an integer
overflow of the unneeded multiplication.

Anton Khirnov 77eb5504d3 avio: avio: avio_ prefixes for put_* functions 
In the name of consistency:
put_byte           -> avio_w8
put_<type>         -> avio_w<type>
put_buffer         -> avio_write

put_nbyte will be made private
put_tag will be merged with avio_put_str

Diego Elio Pettenò c6610a216e Prefix all _demuxer, _muxer, _protocol from libavformat and libavdevice. 
This also lists the objects from those two libraries as internal (by adding
the ff_ prefix) so that they can then be hidden via linker scripts.

Reimar Döffinger 43d7c6118d put_bits can only reliably write up to 31 bit bits, above it relies on 
undefined shift behaviour.
Document this, fix the assert and add a put_bits32 to handle writing 32
bits and use that where necessary.

Ronald S. Bultje f4ca612fbd Set restrictions on packet_size, as per ISO-11172 / H-222 specifications 
(max packet size should fit in 13 bits as a kB value, so 1<<23, plus the
header which is 10 bytes), and as per mpegenc.c internal assumptions
(min packet size is 20 bytes). See "[PATCH] make packet_size in
AVFormatContext unsigned" thread.
