Andrew Stone 7e38903b5c http: enable icy metadata by default. 
It won't hurt servers that don't care about the header,
and those that do will include it by default.

Andrew Stone afbd4b7e09 lavf: add AVFormatContext/AVStream fields for signaling to the user when events happen. 
The only flags, for now, indicate if metadata was updated and are set after each call to
av_read_frame(). This comes with the caveat that, on stream start, it might not be set properly
as packets might be buffered in AVFormatContext.packet_buffer before being given to the user
in av_read_frame().

Diego Biurrun ad1ee5fa75 avcodec: Undeprecate reordered_opaque 
It allows attaching other external, opaque data to the frame and passing it
through the reordering process, for cases when the caller wants other data
than just the plain packet pts.  There is no way to cleanly achieve this
without the field.

John Stebbins 0897d2fdc7 movenc: Add option to disable nero chapters 
And add flag to muxer documentation.
Nero chapters break some taggers (mp3tag and iTunes).

Vittorio Giovara e96c3b81ca avutil: rename AV_PIX_FMT_Y400A to AV_PIX_FMT_YA8 
The rationale is that you have a packed format in form
<greyscale sample> <alpha sample> <greyscale sample> <alpha sample>
and shortening greyscale to 'G' might make one thing about Greenscale instead.
An alias pixel format and color space name are provided for compatibility.

Anton Khirnov 194be1f43e lavf: switch to AVStream.time_base as the hint for the muxer timebase 
Previously, AVStream.codec.time_base was used for that purpose, which
was quite confusing for the callers. This change also opens the path for
removing AVStream.codec.

The change in the lavf-mkv test is due to the native timebase (1/1000)
being used instead of the default one (1/90000), so the packets are now
sent to the crc muxer in the same order in which they are demuxed
(previously some of them got reordered because of inexact timestamp
conversion).

Anton Khirnov 894682a973 Remove avserver. 
It has not been properly maintained for years and there is little hope
of that changing in the future.
It appears simpler to write a new replacement from scratch than
unbreaking it.

Anton Khirnov 0957b274e3 lavc: add an option to enable side data-only packets during encoding 
Some encoders (e.g. flac) need to send side data when there is no more
data to be output. This enables them to output a packet with no data in
it, only side data.
