Anton Khirnov fb722a900f avconv: remove -same_quant 
It has not worked for anything other than fringe codecs (asv1/2, mdec,
mjpeg[b]) since about 2003 and nobody ever noticed or complained. This
sufficiently proves that there are no users of this option who have a
clue of what they are doing, so it is completely useless.

Anton Khirnov 79600a8354 avconv: deprecate -isync. 
This option does not do anything.

Also remove OPT_GRAB, since -isync is the last option using it.

Anton Khirnov 832ba44d8d avconv: saner output video timebase. 
r_frame_rate should in theory have something to do with input framerate,
but in practice it is often made up from thin air by lavf. So unless we
are targeting a constant output framerate, it's better to just use input
stream timebase.

Brings back dropped frames in nuv and cscd tests introduced in
cd1ad18a65

John Van Sickle d6a77e2b97 docs: use -bsf:[vas] instead of -[vas]bsf. 
The latter syntax is now invalid.

Anton Khirnov b2254d8385 avconv: move frame pixel format to the options context. 
Also remove redundant -pix_fmt list syntax, it's inconsistent and  only
makes the code more complicated. -pix_fmts should be enough for
everyone.

Anton Khirnov 77d9c4542c avconv: rewrite -qscale and -aq handling. 
Merge video_qscale (set by -qscale) and audio_qscale (set by -aq) into
one 'qscale' field in the options context. Add a shortcut -q for -qscale
and make -aq an alias for -q:a.

Anton Khirnov d821cbe2ff avconv: move bitstream filters to options context. 
Change syntax -- -[vas]bsf are replaced by -bsf:[stream specifier], the
argument is now a comma-separated list of bitstream filters.
