Changes include removing keystone and putting it into a separate document,
removing user config and putting it into an admin guide, and creating
separate config examples for CentOS/RHEL and Debian/Ubuntu. Needs
clarification on chown/chmod.

This reintroduces the same semantics that were in place in dumpling prior
to the refactoring of the cap/command matching code.
We haven't added this requirement to auth read-write operations as that
would have the potential to break a lot of well-configured keyrings once
the users upgraded, without any significant gain -- we assume that if
they have set 'rw' caps on a given entity, they are indeed expecting said
entity to be sort-of-privileged entities with regard to monitor access.

Following an IRC discussion, it emerged that it would be helpful
to explain the merit of choosing a number of PGs per pool that is
a power of two, to keep PGs at roughly equal sizes in case of
PG splits.

Include erasure code information. A choice is made to display properties
that should be documented as part of the erasure code plugin
interface (erasure-code-k for instance). However, there only is one
plugin at the moment. Furthermore, erasure-code-k and erasure-code-m are
the two properties that the system administrator will want to tweak when
playing with erasure coded pools for the first time. It is probably
better to document them in a very visible place for this reason.

This is for the MDSTable subclasses: AnchorServer,
SnapServer, InoTable. AnchorServer was partially there
but didn't work in practice dumped objects because of
the version prefix on the object.
This adds a skip argument to ceph-dencoder for getting
past fixed size prefixes. The encode_server_state method
is also made const to avoid the need for const_casting (and
because it really is const)

Explain why people should be using the "raw" image format for RBD
volumes created for use by QEMU: using any other format adds only
overhead, but no extra value (since RBDs are also CoW and
thin-provisioned), plus the Qcow2 storage driver is not migration safe
when caching is enabled, whereas the RBD driver is.
Also, fix a minor glitch in the example qemu-img commands ("-f rbd"
and "-O rbd" should really be "-f raw" and "-O raw").
Finally, drop the "-f" option altogether on qemu-img commands where it
makes no sense (info and resize).
