Added note about the requirement for the latest ceph-iscsi version
3 to the dashboard documentation. Added some doc references
and replaced some URLs in the iSCSI docs with reST labels instead.

The config file paths listed in the docs don't match the config file
paths actually searched by ceph (see `src/common/config.cc`). Update the
docs to reflect the current reality.

Since kraken, Ceph enforces a 1:1 correspondence between CRUSH ruleset and
CRUSH rule, so effectively ruleset and rule are the same thing, although
the term "ruleset" still survives - notably in the CRUSH rule itself, where it
effectively denotes the number of the rule.
This commit updates the documentation to more faithfully reflect the current
state of the code.

The docs had the recommended journal size based on the option
"filestore min sync interval" when it should have been
"filestore max sync interval".
While in there, fix a couple of typos -- multiple when it should
be multiply, and a missing word. Change "Should at least twice"
to "Should be at least twice..."

- Added a [client] entry so people know it can be set too.
- Updated existing auth example.
- Added an authentication section with a link to the cephx guide.
