btrfs-progs: mkfs: rework --subvol CLI option

Change mkfs.btrfs --subvol so that instead of being of the form --subvol
DIR:FLAGS, it's instead --subvol MODIFIER:DIR, with MODIFIER being ro,
rw, default, or ro-default.
