685bbb5734 DOCS/xml/en: remove various outdated documentation 
The XML documentation was badly outdated, with various obsolete,
useless and and sometimes actively harmful advice. Delete a lot of the
obsolete stuff, without spending much effort to replace it for now.

In the FAQ section I removed some questions completely; in other cases
I only removed the answer if I thought the entry might be worth
keeping but the answer would need to be updated.

a780989420 convert DOCS/xml/en and help_mp-en.h to UTF-8 
(running help_mp-en.h through iconv didn't change the file so only the charset
file needed to be changed)

fc76820d00 add some internal links between "codecs supported by mencoder" and 
"codecs featured by lavc". Added an audio encoding example.
Moved the "codecs featured by lavc" in 2 shiny new sections (to allow
the internal links to work)

df8fcb57e5 re-organize MEncoder doc in a more sensible way: splitting "basic mencoder usage" and "encoding with mencoder". 
note: if you can't generate the doc on your machine, make sure you run "make distclean" beforehand
