b0986b3760 Merge svn changes up to r30463 
Note that r30455 is wrong, that commit does not in fact change the
default behavior as claimed in the commit message. It only breaks
"-af-adv force=0", which was already pretty much useless though.

4722fa0b82 IWMMXT optimizations were removed from our internal libmpeg2 copy, so now 
remove it from the library interface code as well.

31db39d8ec Make video decoder description structs const 
This makes the per-decoder struct vd_functions and its ->info
struct constants. Same for the mpcodecs_vd_drivers[] table of
pointers to those structs.

12882989fc skip null frames in mpeg files, patch by Zoltan Hidvegi <mplayer@hzoli.2y.net> 
I have an mpeg2 avi (created with mencoder -oac copy -ovc copy from a
dvd) that has null frames, probably inserted by mencoder.  I can play
that with -vfm ffmpeg, but mplayer dump when played with the default
libmpeg2, the trivial patch below fixes that.

48ecb3e717 clean up field flags: 
1) cosmetic change, no reason these need to be mpeg2-specific
2) add a flag to tell when fields are ordered, so we don't have to
assume bff (which would usually be wrong) when field flags are not
available.
3) add other flags for future use :)
