vo_opengl: implement videotoolbox hwdec on iOS

Implements --hwdec=videotoolbox on iOS. Similar to hwdec_osx.c, but
using CVPixelBuffer APIs available on iOS instead of the equivalent
IOSurface APIs in macOS.
