Fixed size given to init_get_bits().
...
init_get_bits() takes a number of bits and not a number of bytes as
its size argument.

Add ff_ prefix to data symbols of encoders, decoders, hwaccel, parsers, bsf.
...
None of these symbols should be accessed directly, so declare them as
hidden.

Split the ADTS header decoder off of the ADTS parser.
...
The AAC decoder and ADTS-to-ASC BSF both require the header decoder
but not full parsing capabilities.
