Reimar Döffinger 371e165443 Try to set AVFrame.reference to correct values. 
I am not sure these new values are correct, not am I sure
the semantics are a good idea since we do not seem to make any
use of them but they caused a lot of confusion, but this
seems to make things closer to matching the documentation.

Laurent Aimar 69a0bce753 Fixed deference of NULL pointer in motionpixels decoder. 
Some of the arguments given to init_vlc() come from the stream
and can be corrupted.

Stefano Sabatini 01042d4123 lavc: set defaults in internal codec frames 
This is required specifically for setting frame->format to -1,
otherwise it will be set to 0 = PIX_FMT_YUV420P and code reading
the format from the output decoded frame will get misled.

In particular fix regressions occurring with the pending vsrc_buffer
patch.

Reimar Döffinger 7c7d3abf3c 10l, pix_fmt should be set by the motionpixels decoder, not by the demuxer. 
This fixes playback when lavf and lavc are less tightly couples as in e.g. MPlayer.

Thilo Borgmann 7a00bbad21 Implement avcodec_decode_video2(), _audio3() and _subtitle2() which takes an 
AVPacket argument rather than a const uint8_t *buf + int buf_size. This allows
passing of packet-specific flags from demuxer to decoder, such as the keyframe
flag, which appears necessary to playback corePNG P-frames.

Patch by Thilo Borgmann thilo.borgmann googlemail com, see also the thread
"Google Summer of Code participation" on the mailinglist.
